Your job is harder than “find a provider”
As a Support Coordinator, your responsibility isn’t simply to find an allied health practitioner.
It’s to help participants access the right practitioner, someone with the experience, skills and availability to help them reach their NDIS goals.
Finding the right occupational therapist, behaviour support practitioner, psychologist, neuropsychologist, speech therapist, physiotherapist, exercise physiologist or dietitian can take hours of calls, emails and follow-up. Multiply that across disciplines and participants, and valuable time is lost that could be spent supporting people directly.
CARED makes this simple. With a single referral, our clinical team coordinates all eight disciplines, manages the entire matching process, and has every practitioner work from one shared record towards the participant’s goals.
Your participant tells their story once. And every referral comes with tangible evidence that the decision was based on participant needs and clinical suitability, not convenience, demonstrating alignment with NDIS principles of choice and control.

The right practitioner for the right participant
Our role is not to allocate the closest available practitioner. We match each participant with the practitioner whose experience, clinical expertise and interests best align with their disability, functional needs, age, communication style, goals, support requirements and preferred service delivery, whether that's home, community, telehealth or clinic.
Instead of simply assigning the next available occupational therapist, we may recommend a practitioner because of deep experience with autism, intellectual disability, psychosocial disability, acquired brain injury, stroke rehabilitation, progressive neurological conditions, Functional Capacity Assessments, Specialist Disability Accommodation assessments, complex assistive technology or home modifications, whichever your participant actually needs.
Because the best outcomes come from the best clinical fit, not the nearest appointment. Matching expertise to need leads to better engagement, stronger therapeutic relationships and better outcomes.
Helping you demonstrate impartiality
Support Coordinators are expected to uphold participant choice and control while avoiding conflicts of interest. For every referral, CARED can provide a practitioner matching summary that documents:
- Why this practitioner was selected
- Their relevant clinical experience
- Their experience supporting participants with similar diagnoses or functional needs
- How their expertise aligns with the participant's NDIS goals
- How the participant's preferences were considered
This creates a clear record showing the referral was based on the participant's needs, not convenience or provider preference. It gives you confidence that your referral decisions are transparent, evidence-based and participant-focused.
And the decision always remains the participant's.
Under the NDIS, choice and control sits with them: we present well-matched options with clear reasons, and which provider to engage is always their choice. A strong referral process doesn't just find a provider; it empowers the participant to choose the right one for them.

Less administration.
More time supporting participants
Once you confirm your participant's funding and referral requirements, CARED takes care of the rest: practitioner matching, appointment scheduling, participant contact, ongoing booking management, cancellations and rescheduling, waitlist management, and communication between practitioners.
That means less administration, less time on the phone, faster commencement of services, a better participant experience, more efficient use of your Support Coordination hours, and NDIS funding preserved for support rather than searching.
Service standards you can quote to participants
- Referral acknowledged within minutes, to you and the participant
- A human contacts you within hours
- No waitlists: participants can secure appointments today and start within the week
- Progress updates and reports delivered on schedule
- One point of contact for all eight disciplines
Aged care referrers welcome: care partners, Support at Home providers and CHSP coordinators can refer older clients through the same process, with the same service standards.

Collaboration makes it work
This is where CARED practitioners excel.
Evidence shows that participants with complex needs benefit from coordinated, multidisciplinary care where practitioners work collaboratively towards shared goals.
Compared with fragmented service delivery, integrated approaches improve continuity of care, communication between providers, participant experience and access to services. Importantly, these benefits arise from effective collaboration, not from limiting participant choice, and are achieved while fully respecting NDIS principles of choice and control.
